1. Program (driver) can be loaded from registry, for example
local_machine/system/currentcontrolset/services/vxd/agent 95/agent95.vxd
loads this VXD at startup.
Registry Search program can be used to find these renegade drivers.

11. All you need is right-click on PIF-file, then click "Properties" -- you'll have access to all PIF settings.
BTW, the PIF file in terms of Win95 is exactly the same that "Shortcut" to DOS program.
